Terms – Registration Requirements :

1). GENERAL REQUIREMENTS

Attach a letter of application as a participant in the Direct Selection to participate in the procurement process addressed to the Procurement Manager.
Attach a photocopy of the Business Place Permit (SITU) / Certificate of Company Domicile.< br />Attach a photocopy of a valid Company Registration Certificate (TDP).
Attach a photocopy of a valid Trading Business Permit (SIUP).
Original Bank Reference from a national commercial bank.
Attach a photocopy Taxpayer Identification Number (NPWP)
Attach a photocopy of the Taxable Entrepreneur Confirmation Letter (SPPKP).
Attach a photocopy of the company's 2014 financial statement/balance sheet.
Attach a photocopy of the Company's Deed of Establishment and the Ministry ofHuKam's endorsement sheet/ Minister of Keh.
Attach a photocopy of the Deed of Change of Company and the Approval Sheet from the Minister of Law.
Attach a photocopy of the Identity Card (KTP) of the Company Management which is still valid.


2). SPECIAL TERMS

1. Have experience in similar work in the last 5 (five) years for the transportation of BBM/BBG by attaching a list of work experience as evidenced by 1 (one) copy of the contract and 1 (one) copy of the 100% Work Completion Minutes.

List of work experience should at least contain the following information (example of list of Work Experiences attached):

- Name of project/job

- Location of project/job

- Name of assignor/project owner

- Value of work/contract

- Duration of implementation

- Status in carrying out the work at that time (main contractor, sub contractor or supplier).

2. Have a Pertamina Contractor Safety Management System (CSMS) certificate with the classification or category: High Risk as evidenced by a photocopy of the certificate. Or for companies that do not have a CSMS Certificate, they are required to fill out the CSMS form and its supporting completeness according to the attached form (it will be assessed by the Committee from the HSE function, and the results must meet the High Risk category).

3. For companies that register, they must make a Power of Attorney from the company with a stamp duty of Rp. 6,000,- (except for the Director/Vice Director).

Note :

One umbrella company applies the following provisions:
1. Not included in the group of companies whose capital ownership the majority is owned by the same person/owner to follow the same procurement process.
2. Has no affiliation, namely a relationship between 2 (two) or more companies where there is 1 (one) or more of the majority shareholders/capital owners the majority, members of the same Board of Directors or Commissioners to participate in the same procurement process.
All registration documents are arranged systematically in a neat format, complete with cover, table of contents, and boundaries between documents.
For registrant companies, if the registration documents do not meet the pre-qualification requirements, they cannot be invited to take part in the next stages of the procurement process.
This procurement process refers to the Decree of the Board of Directors of PT Pertamina Gas No. Kpts – 055/PG0000/2013-S0 regarding the Procurement of Goods and Services.
